Subject Access Request
Summary
Welcome to our access to information portal.
You and/or third parties authorised to act on your behalf, can use this portal to ask us for copies of the personal information we hold about you. This is known as a 'Subject Access Request' (SAR) under current data protection law.
We use this portal to process your request and securely send information.
There is no charge for submitting a SAR but we can charge a 'reasonable' fee if we feel that the request is 'manifestly unfounded' or 'excessive'. Fees are based on the administrative costs of providing the information.
You will receive your information within one calendar month from the date of request. If your request is complex, we are allowed to extend this a further 2 months but we will tell you if this is the case as soon as we can.
We may not be able to supply all the information you have asked for. It may be that you only receive part of it, or none of it, depending on the circumstances. In such cases, we will tell you within one month and give you our reasons why.
We need to be sure who you are so you must provide two forms of identification; one to tell who you are (passport, driving licence, national identification card) and one to tell us where you currently live (council tax bill, bank/credit card statement, utility bill).
For requests made on behalf of someone (living or deceased), you must provide your identification and one of the following:
On behalf of | Document required |
Child or young person | Copy of the child's full birth certificate |
Deceased person | Copy of the Will stating that you are the executor of the deceased's estate; or Probate form stating that you are the deceased's personal representative |
Living person | Signed letter of consent from the individual that confirms authorisation to act on their behalf; or Court Order stating that you have power of attorney for Health and Welfare |

If you would like to request a copy of the personal information we hold about you, please create an account, log into the portal and complete the relevant online application using one of the forms listed below:
1. Application for own personal data.
2. Application for requesting information on behalf of others: Relatives, Friends, Advocates.
3. Application for requesting information on behalf of Professional Organisations: Police, Solicitors, MOD, NHS Trusts.
4. Application for deceased health records.
